How Medicare penalties work

Part B: 10% of the standard premium ($202.90 in 2026) for each full 12-month period you delayed. Permanent.

Part D: 1% of the national base premium ($38.99 in 2026) for each month without creditable drug coverage, rounded to the nearest $0.10. Permanent.

How to avoid penalties

  • Enroll during your Initial Enrollment Period (7 months around your 65th birthday)
  • If working past 65 with employer coverage (20+ employees), your 8-month SEP protects you
  • Keep proof of creditable drug coverage from your employer
